Client: Non Profit Contract from June -August (possibility of extension but is covering for a LOA so it would depend on if there is another opening within the budget at that time) STARTING ASAP! The
Program Manager leads the
Connect2Care (C2C) program, which helps low-income, chronically ill New Yorkers access health and social services. The program focuses on improving access to preventive care, screenings, and support services, targeting issues like diabetes, heart disease, maternal health, obesity, and hypertension.
The Program Manager works closely with
community-based organizations (CBOs) to build their capacity to deliver culturally competent health services. This includes training
Health Advocates , supporting onsite health services, and ensuring partners can meet grant goals and reporting requirements.
They will coordinate with CBOs, healthcare providers, and other stakeholders to manage the program, support implementation, and monitor performance through regular reporting and site visits.
Key Responsibilities
Serve as the main contact for C2C partners; provide guidance and technical assistance.
Help partners meet grant requirements, including submitting reports and managing budgets.
Increase access to preventive health services for underserved communities.
Build and maintain relationships with CBOs to support health equity efforts.
Research best practices in community health programming and apply findings.
Create outreach and training materials; identify training needs for partners.
Train partners on data systems and proper submission of screening and referral data.
Collaborate with the Senior Director to submit invoices and report spending.
Monitor partner performance through site visits and report reviews.
Use the Results-Based Accountability (RBA) framework to track outcomes.
Stay up to date on local and national health policies.
Support grant administration and other tasks as needed.

Communicate change clearly to support team engagement.
Work with leadership to identify opportunities and implement strategies.
Qualifications
Master's degree in social work, public health, public administration, or a related field and 2+ years of experience

Bachelor's degree in related fields with 4+ years of relevant experience.
Experience in grant management , program administration , or preventive healthcare .
Strong skills in relationship-building , teamwork , and collaboration .
Excellent communication, presentation, and facilitation skills.
Highly organized, detail-oriented, and self-motivated.
Tech-savvy and quick to learn new systems; proficient in Microsoft Office Suite .
Experience working with CBOs or in grantmaking is a plus.
